To join our multidisciplinary team you will:

– be a qualified social worker with current professional registration
– be proactive and adaptable
– have a passion for working in partnership with service users and their families to support progress through their pathway to discharge
– be able to take an active role in safeguarding within the multidisciplinary team
– be able to lead on assessing and supporting the social care needs of patients
– be seeking a new challenge

The post involves a 7 day working rota.
