The Nottingham City Street Outreach Team is looking for a qualified and registered Social Worker with at least one years’ experience post qualification (applicants will have successfully completed an ASYE).

The main duties will include:

• Carrying out direct work with people sleeping rough and working closely with the local authority and other partners to ensure the rough sleepers have access to the best possible support and advice. Where appropriate the post holder will work alongside the Local Authority to coordinate assessments under the Care Act 2014 with people sleeping rough.
• Acting as a main point of reference regarding the most vulnerable rough sleepers in the Nottingham City area. In particular regarding people who may have issues associated with their mental health or ongoing safeguarding concerns.
• To be available to the Street Outreach Team to either advise or review cases of vulnerable rough sleepers supported by the team (and potentially take on to own case load)
• To take a lead role in liaising with statutory agencies where the Street Outreach Team encounters individuals about whom there are potential safeguarding concerns.
• Empower service users to contribute to their own support by using expert knowledge of legislation and locally available services to identify realistic, achievable outcomes
• Undertake street outreach sessions when required (which may take place outside normal working hours) to engage rough sleepers and maintain contact with current caseload.
• To case work rough sleepers with identified care and support needs, as defined by the Care Act.
o To accurately record all interactions using the SOT Client Database
o To communicate effectively with relevant statutory and voluntary sector partners in supporting and advocating for individuals case worked
o Assess clients face to face in a community setting in order to establish personal needs and circumstance that will form the basis of a support plan or onward referral
• To attend and represent the service at Multi Agency forums and events that take place in the Nottingham City area relating to rough sleeping and to produce reports and case studies as required.
• To build and maintain relationships with voluntary and statutory partners
• To keep updated with National and Local Policy in relation to Adult Social Care and implications for homeless and vulnerable people
• To contribute at weekly Team Meetings and Team Development Days and to update the Street Outreach Team on policy and implications.
• Compose, deliver and be held accountable for the production and distribution of information in the form of Daily Reports, data collection and accurate running records – informing external and internal partners of current cases.
• Advise, signpost and emotionally counsel where appropriate all incoming telephone calls to the Freephone telephone number – the most widely advertised and visible means of public contact for the organisation.
• Play an active role in all Framework’s activities relating to social work education and professional
• The post holder will possess good time management skills.
• The post holder will possess an understanding of the issues faced by homeless people such as substance misuse, poor mental health, offending and domestic abuse.

Use of own car and full UK driving license is not essential.
