An exciting opportunity has arisen for a highly experienced and motivated social worker to join our patient and family support services at our hospice in Hampstead, central London, to provide support within our in-patient unit and our local communities.
Ideally you will have experience of working with people who are dealing with end of life care issues and living with a life limiting illness. As a result of the pandemic the hospice is exploring how we best serve our patients and their families.
With a rapidly changing shift in the health system and health economy in the COVID crisis we are redesigning our services and this role will offer the successful candidate opportunities to provide an innovative virtual service and expand our input within our local communities.
The post holder will provide excellent psychosocial support to patients and their families known to the hospice. The post holder will support developments within the patient and family support team and work closely with clinicians and managers in ensuring relevant national guidelines are followed and implemented, the safeguarding of both adults and children, supporting the delivery of high-quality care.
The successful applicant will have the right qualification and experience including working with patients and their families in health care settings and in their communities using a broad range of social work skills. You will be an effective communicator with good interpersonal skills. In return we offer you the opportunity to work within our supportive teams, which promote and support ongoing development.
What we are looking for:
– Excellent communication skills, with the ability to work as an autonomous practitioner, as well as a team player
– Knowledge and understanding of palliative care
– Degree in Social Work or equivalent
– Experience of working as part of a multi-disciplinary team
