The role of the care coordinator is to work collaboratively with service users and carers. This may include psychological interventions, medication support, access to group work and carers support. You will engage service users creatively and develop collaborative care plans including risk management plans. We are looking for enthusiastic, innovative mental health practitioners either from a background in Mental Health Nursing, Social Work or Occupational Therapy who are committed to the principles of recovery. We are a small, friendly team who believe in working together and supporting each other in managing complex cases. Southern Health is one of the largest Foundation Trusts in the UK, specialising in mental health, adult and child community health and learning disabilities. We are committed to promoting the welfare and safeguarding of children, young people and adults at risk of abuse and neglect through our 6,500 strong workforce
