We are looking for an enthusiastic and adaptable Health and Social Care Coordinator to join our strong and progressive East Kent team.
You will be based at Queen Victoria Memorial Hospital, Herne Bay and will be required to travel across East Kent.
You will support adults with long term conditions living primarily in the East Kent area by enabling them to access and engage with wider health and social care sectors to improve their overall health and wellbeing, by aiming to improve self management and unnecessary hospital admissions due to crisis.
You will share the CARE values of the trust and work within a close-knit group of people who bring the feeling of community to the team through their hard work and dedication to the people of the area.
The Health and Social Care Coordinator Team consists of 4 team members that deal with incoming referrals between the hours of 0900-1700 Monday to Friday.
Main duties of the job
The Health and Social Care Coordinator role is integral in the development and coordination of the Community Hub Operational Centres.
You will be responsible for ensuring that patients are referred on to the most appropriate health, social and/or voluntary organisations in a timely manner. A telephone call or face to face assessment in the patient's home environment may be required to ascertain the needs of the patient.
