You will be working as part of the MDT who provide support and treatment to people both within the hospice and the community setting.

The successful candidate will work closely with and develop relationships with statutory services to provide successful outcomes for the patients and their families.

The post holder will be required to develop student and volunteer opportunities within the organisation and provide supervision to these.

You will need to be a confident communicator and calm decision maker, you'll need experience and knowledge in complex patient management, together with well-developed assessment, communication, and planning skills. The ability to prioritise your work, whilst juggling a variety of changing demands, is vital.

Ability to travel across the locality will be required, therefore the ability to drive with appropriate business insurance is essential.
