Main duties of the job:
You will be assessing and responding to complex needs, working with risk and uncertainty whilst demonstrating your ability to promote enablement and self-determination within this. You will have extensive experience, skills and knowledge of working in partnership with people to achieve the personal outcomes they seek, and an ability to develop trusting relationships to support individuals facing complex social and emotional challenges.

- Experience in adult care
- Proven experience of assessment and planning intervention
- Proven experience and understanding of the professional roles and the contribution of all members of the multi-disciplinary team
Abilities and skills
Essential criteria
- Good knowledge of Microsoft Applications and ability to use IT based systems
- Good communication and interpersonal skills and to be able to use these effectively in a variety of settings
Desirable criteria
- Understanding of the needs of unpaid carers
- Knowledge of current legislation relevant to role
- Knowledge of relevant voluntary and statutory agencies
