A vacancy has arisen in the Older People's Mental Health Team based in the Chadwell Centre Paignton for a registered Social Worker.

This is a 12 month Secondment Opportunity of 37.5 hour per week post in a multi-disciplinary co-located Health and Social Care Team which has an excellent reputation for service provision to older people with functional and organic mental health difficulties and their carers.

Main duties of the job

Ideal applicant will have experience and knowledge of working with individuals with organic and functional mental health issues. Have previous experience of working within the Care Act; Mental Capacity and Mental Health Act; understanding of Deprivation of Liberty Safeguards and Safeguarding Adults.

Your practice should be Strengths Based (Respect and Dignity) and have experience of undertaking holistic needs-led assessments in partnership with service users, patients and carers. Developing care plans that focus on strengths whilst drawing on the expertise of the service user, the family unit and community-based resources.

Demonstrate professionalism and integrity, being a role model to those around us and ensuring everyone has an equal opportunity. You must have excellent communication, organisational, assessment and care planning skills are essential to the work with service users with complex mental health and social care needs and their carers.

Essential criteria

• Relevant professional Social Work qualification – Degree/Diploma Work, Certificate of Qualification in Social Work, Certificate in Social Services.

Desirable criteria

• Post Qualifying consolidation and commitment to post qualification training

Knowledge and Experience
Essential criteria

• Working knowledge of Care Act, Mental Capacity Act. • Working knowledge of the Care Act • Understanding of Mental Health Act
• Ability to present as an excellent role model for good practice within • The multi-disciplinary team • Ability to take a lead role in jointly-worked complex cases • Ability to think laterally whilst maximising quality outcomes from resources • Ability to work on own initiative while making positive use of supervision and training • Ability to use IT-based Care management systems • Ability to make decisions in accordance with the statutory requirements of the post • Ability to manage time effectively in assessing, planning, monitoring and reviewing service user’s needs and services
• Good reporting and recording skills • Ability to communicate verbally and in writing with a broad range of people • Commitment to the concepts of a needs-led service, quality standards and service user choice within the context • Anti-discriminatory practice. • Ability to work within corporate policy and an awareness of Equal Opportunities

• Health and Safety in the workplace legislation • Ability to undertake risk assessments within all aspects of day to day work. • Ability to act as a consultant on practice-based issues for other health and social care practitioners
• Knowledge of safeguarding adult processes and No Secrets framework • Knowledge of Continuing Care/ Registered Nursing Care • Contribution • Knowledge of Direct Payments and Independent Living Fund • Knowledge of Intermediate Care
