Exciting opportunities has arisen within the South East Unplanned Service for two registered Level 1 Social Workers.

The position is within theolder persons with physical disability teamwhich is a community based team working with Adults over 18 years of age with a range of physical and mental health needs. This vacancy is open to newly qualified Social workers.

The postholder's will be registered Social Worker's and will have responsibility for completing statutory assessments to enable timely and safe discharges from Hospitals. Tamworth and Lichfield Community Hospitals are based at Samuel Johnson Community Hospital Lichfield and Sir Robert Peel Hospital, Tamworth. It will be the postholder's responsibility to work with the multi-disciplinary team to ensure safe and timely Hospital discharge.

Main duties of the job

Accountable on a day to day basis to the Advanced Social Work Practitioner, who will allocate, monitor and review all work through regular supervision.

Supporting the delivery of both the personalisation and social inclusion agendas ensuring quality public facing services including health and social care services. Involving assessing; accessing; and implementing a range of social care services, and providing guidance/advice and information for adults with acute or long term physical health, mental health needs and for older people and carers.

To be an integrated and supportive member of a team, planning, actioning, monitoring and evaluating work with the supervision of an experienced Social Workers.

Supporting the delivery of health and social care functions for adults/carers within the Community and where relevant Hospital/ Inpatient settings providing social work knowledge and expertise within the domain of assessments, care and support planning, reviews and requests for information, advice and guidance.

Responsible for working in partnership with agencies external to the Trust supporting adults/carers to access services/support in the independent and non-statutory sector.

Responsible for upholding Trust Quality standards including Customer Services quality standards to all callers including adults/carers new and/or known to the service, and for obtaining and recording with care and providing up to date/accurate case and performance recording requirements.

Social Work Learning Academy (SWLA)

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Person Specification
Qualifications & Training
Essential

- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
- Ensuring current registration with the Professional Social Work regulatory body at all times
- A commitment to on-going personal and professional development
- To undergo appropriate training to achieve Assessed Year in Practice (ASYE)
- Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
- A clear understanding of and commitment to equal opportunities and anti-discriminatory practice

Experience
Essential

- Experience of working with Adults and older Adults with mental health needs and their carers

Desirable

- Experience of social work in other user groups/settings i.e. Children and Families, Learning Disability, Mental Health
