This is an Advanced Mental Health Practitioner role within a busy Crisis Resolution and Home Treatment Team. The successful applicant will have the opportunity to lead a small team and liaise directly with a variety of other professionals and agencies to gather the necessary clinical information to support their clinical decision making and an informed assessment to patients in mental health crisis.

They will work directly with the Crisis Resolution and Home Treatment Team to offer a specialist response to people who are at their most vulnerable and experiencing a mental health crisis. In addition, the successful applicant will hold excellent verbal communication skills and have the ability to support other agencies with referral options and clinical decision making.

They will take responsibility for managing the clinical caseload as well as supervising a small staff team they will work across CRHTT West ensuring continuity of service out of hours.

Main duties of the job

- To have the key skills and competencies to provide high quality Psycho-Social assessment with the ability to undertake robust risk assessment and gatekeeping in the community?
- To have a level of commitment and drive to lead a small team in providing Home Treatment to people during this difficult period in their lives
- Working in a team of professionals caring for those in mental health crisis.
- Work flexibly in response to service needs across multiple sites.
- Take responsibility for case management of those with Complex MH needs.
- Supporting colleagues in the delivery of compassionate, safe and structured care
- Plan and provide treatment in collaboration with others as appropriate

- Experience of working with people in MH crisis
- Ability to effectively manage and lead a small MDT.
- Car driver with the ability and readiness to travel to multiple sites
- The service is a 24/7 operation so flexibility is key to this role

Person Specification
Education/Qualifications/Training
Essential

- Degree in health/social care or equivalent qualification/experience
- Knowledge and experiencing of managing situations concerning safeguarding, risk management, complaints and investigations
- Registered Mental Nurse, Occupational Therapist, Allied Health Professional or Social Worker whereby the person is licensed to assess, plan, implement and evaluate care for patients with mental health problems
- Experience of operating successfully within the Care Programme Approach
- Substantial experience of managing complex caseloads and applying a range of interventions
- Experience of working effectively in a multi-disciplinary team and multi-agency environment
- Specialist knowledge acquired through training in a specialised discipline of healthcare, preferably mental health, although not essential

Desirable

- Experience of clinically supervising junior and unqualified staff and mentoring students, qualification attained in mentoring would be ideal but not essential
- Recognised management qualification would be of benefit although not essential

Continuous Professional Development
Essential

- Evidence of ongoing professional development through attendance at non Mandatory Training events, workshops, seminars, conferences.

Desirable

- Evidence of identifying personal development needs within the Appraisal process or supervision.

Previous Experience
Essential

- Experience of working effectively in a multi-disciplinary team and multi-agency environment

Knowledge, Skills & Abilities
Essential

- Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs
- Effective and competent interpersonal, influencing, written and verbal communication, observation and reporting skills
- Competent IT skills and the ability to navigate around various systems and software packages (such as outlook, databases, MS office and the internet)

Desirable

- Ability to work effectively in a team, work on own initiative, work unsupervised and manage own workload

Additional Requirements
Essential

- Ability to travel to multiple sites
- Manage difficult situations with service users that have complex mental health conditions and challenging behaviours
