Job overview
As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from Kent a and Sussex in specialised mental health care settings, within the NHS and Independent sectors
To manage the cases of service users in Adult Eating Disorders services, ensuring the values and principles of Care and treatment for all service users are integrated into the work of Specialised Commissioning and NHS England.
To provide clinical support to Provider Collaborative in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
