Senior Social Worker and AMHP

We are currently recruiting a Senior Social Workers preferably three year’s post qualification experience. This is a full time vacancy in a hospital team. The successful candidate is someone who is motivated, can work under pressure and is able to demonstrate the benefit of having social work within an acute mental health setting supporting discharge.

The post holder will be part of the local inpatient team and the trust wide Community Access Service.

The post holder would be expected to provide client-centred Social Care assessments including CAA and Carers assessments for service users with complex needs, within the local services working within a multi-disciplinary and social work team.

Provide advice, support and protection as needed

Provide assessment upon access to the services that help keep adults-at-risk safe.

Forge links with partner agencies and safeguarding leads in other areas as well as developing clearly understood pathways for safeguarding referrals from MASH into other Adult Social Work teams will be a major part of each role.

- Key tasks and accountabilities are intended to be a guide to the range and level of work expected of the post holder. This is not an exhaustive list of all tasks that may fall to the post holder and employees will be expected to carry out such other reasonable duties which may be required from time to time
- To participate in a full and flexible manner in the provision of services to vulnerable adults with complex mental health support needs and their carers, using appropriate resources of the Health Trust, the council and the local community, non-statutory organisations and primary care
- To participate in the assessment or care management service in conjunction with other professionals in the team. To participate in the Approved Mental Health Professional rota, ensuring that an effective and sensitive service is provided at all times, with full documentation logged and distributed as appropriate.

Detailed job description and main responsibilities

CLINICAL RESPONSIBILITIES
- To deliver, or support the delivery of, highly specialist evidence-based assessment, care planning, treatment and review for patients admitted on an acute ward

- To provide specialist consultation, advice and support to both WLT in-patient and community services, as well as other external services/stakeholders on best practice when working individual’s on an acute setting.

- To work jointly with care co-ordinators from local MINT Services, with a small complex and challenging caseload with complex social needs

- Within the Acute Service is responsible for promoting and safeguarding the welfare of adults and children who come into contact with the service, in compliance with The Care Act 2014 and Children Act 2004 and ensure that the principles are embedded in all practices, advice, decisions and support associated with this role.

Knowledge
Essential criteria

- Mental health diagnoses/presentations
- Ability to articulate and apply relevant experience to mental health setting
- Core skills of housing and social care process
- Clinical and environmental risk

Desirable criteria

- Relevant current legislation, NICE guidelines, CPA, risk assessment and risk management
- Evidence of training in Mental Health
- Clinical/case supervision training

Qualifications
Essential criteria

- Qualified Social Worker (Registered with Social Work England) evidence of registration
- Evidence of continuing professional development relevant to role
- Current / up to date entry on the mentorship register (or equivalent)

Desirable criteria

Educated to degree level or equivalent
Dual qualified as social worker, RGN, midwife or health visitor
Qualification / training in additional specialist skills e.g. therapeutic / psychological interventions

Experience
Essential criteria

- Significant post qualification experience working in mental health services
- Experience working with individuals with serious mental illness
- Experience of social Work assessment and interventions
- Experience of working in a complex multi-disciplinary / multi-agency context

Desirable criteria

- Involvement in research and clinical audit
- Experience joint working with a multiagency environment
