Job summary

An exciting opportunity has arisen for a full time enthusiastic, social worker to work in the community branch of the Mental Health Rehabilitation Service, we are based at Shaw House St, Austell Cornwall.

The Community Mental Health Rehabilitation team provides a service across the whole county of Cornwall supporting people recovering from complex and enduring mental health issues. This team operates alongside the inpatient facility Fettle House which is based in Bodmin, allowing the team to follow through a service offering a specific pathway for the patients as they resume/ establish independent lives outside of the hospital setting.

The Community Rehabilitation Mental Health team works collaboratively as part of a wider structure within Cornwall Foundation Trust, this includes carers, families, other specialists e.g., Early Intervention Team, we also assist with the discharges of patients from the acute wards and work alongside housing providers, education, pharmacy.

Main duties of the job

-Deliver person-centred support to patients, families, and partner agencies.
-Apply up-to-date knowledge of legislation and best practice.
-Communicate effectively in challenging, emotionally complex situations.
-Work with individuals managing difficult emotions and behaviours.
-Practice with an anti-discriminatory, strengths-based, trauma-informed approach.

This role demands resilience, strong values, and evidence-based decision-making to improve outcomes and support recovery.

Qualifications

Essential
-Qualified Social Worker
-Registered Social Worker with Social Work England

Experience, Knowledge and Abilities

Essential
-Experienced in delivering care/support to adults with mental health issues
-Knowledge of the Mental Health Act 1983 and Mental Capacity Act 2005
-Knowledge of recovery approaches and supporting daily living skills
-Understanding of mental health issues and the mental health system in the criminal justice and secure services pathway

Desirable
-Experience of supervising/managing people
-Experience of working in a forensic setting
