Our aim is to ensure that people experiencing a mental health crisis are offered assessment and intervention quickly and effectively.

Main duties of the job

The Urgent and Crisis services is situated at the Longley Centre within the Crisis Hub, it is a 24 hour service that works all year round, providing Crisis assessments and Urgent interventions for up to 72 hours for the people experiencing crisis in Sheffield.

The team works alongside the NHS 111 mental health option provision, Home Treatment Team, AMHP team and Decisions Unit. We act in line with the UK triage rating scale in order to provide the right service at the right time for patients in crisis.

You will need to be a RNMH, or social worker with experience of working in a multi-disciplinary mental health setting. Skills in assessing mental health and risk, planning, evaluating, and managing care are essential.Knowledge and understanding of PDR and supervision processes are also essential requirements for the post together with a commitment to the appraiser and supervisor role. Additionally, you will need to possess excellent communication skills, be motivated, able to work independently, and as part of a multidisciplinary team and with other partnership agencies. You should have a positive approach to meeting the needs of our service users and be able to demonstrate continued professional development. There will be further opportunities for training and development .

Job responsibilities

-To triage urgent/crisis mental health referrals from a variety of sources and to establish an appropriate response.
-To offer mental health assessment, formulation and time limited treatment planning for individuals needing an urgent/crisis response.
-To provide a resource of highly specialised, time limited interventions to individuals for up to 72 hours following assessment.
-To work with other disciplines and teams within Sheffield Health and Social Care (SHSC) to ensure service users benefit from a multi-disciplinary approach that best matches clinical need.

This post is an Out of Hours post , 3 x 12.5 hr shifts a week.

Shift patterns are-

Twilight -14.50 to 04.00 hrs with 40 minutes break
Nights - 19.20 to 08.30 hrs with 40 minutes break
Weekend/Bank Holiday day shifts -07.20 to 20.30 hrs with 40 minutes break

-To possess excellent risk assessment and management skills for service users experiencing mental health challenges.

Desirable
-Community Experience

Essential
-Relevant Qualification RNMH or Social Worker
-Understanding of mental health crisis / out of hours work.
-Ability to assess plan, implement and evaluate care for service users within mental health services.
-Evidence of clinical risk management skills.

Desirable
-To have worked in crisis and acute care
-Ability to work autonomously, Confidence in working in isolation, as well as part of a team
-Understanding of Local and national policies
-Ability to network and to manage the interface between organisations involved., utilise and contribute to professional networks

-To have experience in delivery of training to nurses/social workers and other disciplines
