Job summary
As part of an ongoing pilot project one of the Care Groups previously working at part of the South CMHT has now moved to Newfield Green Clinic. This is part of the move to integrate secondary mental health care and work closer with primary care and the voluntary sector within the Heeley Plus Primary Care Network.
We continue to provide ongoing intensive support in local communities leading to a high quality care standard and currently still operate as a mini team supporting residents within the Gleadless & Heeley areas.

Hours Ideally Mon 9 - 5pm, Thurs 1-4:15pm, Fri 9-5pm but negotiable
Main duties of the job
You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered and enthusiastic to both learn and share.
You will also likely to be expected to work as part of the duty system which operates within the general working hours of 09:00 to 17:00 Monday to Friday on a rotational basis. This 'mini team' is supported by other clinical professional and management.

Job responsibilities
The post holder will work within a multi-disciplinary service and core responsibilities will include:
oActing as a Named Worker for service users
oComprehensive assessment of new referrals
oCarrying a varied caseload defined by need
oContributing to a duty system on a rota basis
oContribute to Multidisciplinary service development
Clinical
oCarry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
oTo assess, evaluate and manage clinical risk
oTo formulate a plan of care based on identified need
oDeliver evidence based interventions as appropriate
oReview effectiveness of care plan at regular intervals
oTo work within safeguarding processes, including where required to act as safeguarding investigator / manager.
oTo work collaboratively with service users, carers and partner agencies
oTo maintain computer held care records in an accurate and timely fashion
oUndertake all work in a culturally sensitive manner
oAct in accordance with Care Management System, Acute and Schedule Care Pathways as appropriate.
oDevelop effective working relationships with Primary Care & Voluntary Sector colleagues

Knowledge and skills
Essential
oWorking knowledge and understanding of MHA/MCA
oKnowledge and understanding of appropriate therapeutic approaches and engagement strategies
oKnowledge and understanding of PDR and supervision processes as a supervisee
oDemonstrable risk assessment and risk management skills with evidence of positive risk taking
Desirable
oAdditional skills /training in psychosocial interventions for people with SMI, CBTp and family interventions
oRelevant Health or Social Care professional qualification - Nursing
