Job summary

This is a great opportunity for a highly motivated, dynamic and organised Band 4 practitioner to join the Adult Inpatient Division, Bed Management & Adult Inpatient Social Work Team. The team work closely with Adult Acute, PICU and Rehabilitation wards to help facilitate safe and effective admission and discharge to and from inpatient settings.

Within this fast paced demanding, but highly rewarding role, you can expect to gain a wide range of experience including knowledge and understanding around treatment pathways, available resources for service users leaving hospital environments and develop close working relationships with all support services across Lincolnshire.

This is a role with considerable responsibility; you'll be working alongside senior clinicians to ensure the best possible outcomes for all service users under the care of Adult inpatient Services.

Main duties of the job

-To liaise with the Registered Social Care Professionals and carry out assigned duties delivering a variety of mental health and social care interventions to service users suffering from mental ill health.
-Provide direct social care support including short term social work intervention, rapid reablement and longer-term social care intervention under the direction of a qualified social worker, where applicable for joint-worked cases.
-Provide direct physical, social and psychological care to service users under supervision in accordance with individualised support plans, reporting about the service user's condition to qualified staff and enabling service users to achieve defined outcomes set out in their support plans.
-Exchange factual information with service users, who may have barriers to understanding using reassurance, tact and empathy.
-Contribute to the assessment and analysis of clinical and non-clinical situations, making routine judgments to resolve straight forward problems.

Person Specification

Qualifications

Essential
-Cavendish Care Certificate or willingness to work towards resourced by LPFT
-NVQ 3 in Care or equivalent
-NVQ 3 in Promoting Independence or equivalent or working towards archievement
-Level 4 qualification in health and social care plus experience within a health and social care setting to diploma level

Desirable
-Training in mental health awareness or any other training relevant to this post
-NVQ 2 in Care or equivalent

Experience

Essential
-Previous relevant experience of working with people with mental health problems as a nursing assistant grade or equivalent within a health and social care setting
-Knowledge of risk assessment principles
-Experience in providing support for people in the community
-Experience of working with people with mental health needs

Skills

Essential
-Effective communication with service user, other members of the team and outside agencies
-A patient and empathic approach. Clear and accurate clinical note keeping
-Quick decision making skills- calm approach and have the ability to liaise and consult with others colleagues if required
-Plan and organise their day and ensure that their whereabouts are known
-IT skills
